Need contribution for your fundraiser? Read me first!

Thank you to all of our patients who gave us the opportunity to participate in their fundraisers last year! As our practice continues to grow – and thus the requests for contributions continue to increase – we have decided to change our process in order to serve you better this year.

Effective immediately, we will plan for contributions on a quarterly basis. For example, fundraising applications received by April 1st will be considered for contribution dollars we have set aside for the months of April, May and June. All donations will be contributed on a first-come, first-serve basis. Please reference the following schedule:

Please submit your request by:
January, February and March January 1st
April, May and June April 1st
July, August and September July 1st
October, November and December October 1st

So, what do you need to do if you would like Ozier Orthodontics to contribute to YOUR fundraiser? Just follow these quick steps:

1. Please complete the Request for Donation application.
2. Turn in your application. during your next visit
3. email (info@ozierortho.com), or
4. fax it to us (972.867.0044).

Once a contribution request is processed, you will be notified as to whether or not we are able to participate. Requests for donations are granted on a first come, first serve basis.
